You can also fill out the following Microsoft Word form and email it to the museum at anthromuseum@ucdavis.edu.

If you are considering performing research at the Department of Anthropology Museum, feel free to get in contact with us otherwise before filling out the forms. Please submit request 2-3 weeks ahead of desired date to allow time for processing and scheduling.
